Program Description:

The Housing First Community Coalition (HFCC)Towne Twin Village main objective is to provide housing and wrap around services to the most vulnerable seniors who have been chronically homeless (per HUD definition). HFCC will does this using Housing First (HF) and Permanent Supportive Housing (PSH) principles at a single-site community. The program will provide formal supports such as intensive case management, medical, dental, and mental health services, but also informal supports such as meals, food pantry, barber, nail grooming, foot clinic, clothing, hygiene products, community garden, movies, social events, etc.

"Unduplicated clients" means each client who receives services in the current contract term will only be counted once in this performance measure. The current contract term is October 1, 2022 to September 30, 2023 for FY2023.

Percentage (%) of clients participating in wrap around supportive services

Percentage (%) of clients report an increased sense of security or social support within three (3) months of program entry
